Background[]
Carlo was the only child of Giuseppe Geppetto. However, Geppetto was consumed by work and neglected his son, even going to far as to send Carlo to live at the Monad Charity House (later converted into a boarding school known as The Rosa Estate). Here, a young woman (Sophia) introduced him to a boy named Romeo and the two quickly became best friends. Both boys had dreams of becoming Stalkers when they grew up and frequently trained with one another.
Overtime, Carlo became resentful towards his father for neglecting to spend any time with him. This grew to a boiling point when Geppetto failed to attend Carlo's graduation. Carlo told Romeo that he wouldn't be sad if his father died, but was reminded that he was lucky to have any family at all. In a form of protest and resentment to his father, Carlo gave his necklace to Romeo as a gift to show his friend that he admired him.
Sometime later, Carlo met the Legendary Stalker and pleaded with her to teach him how to use a sword. However, she found this request annoying and ordered Gemini to: "get rid of him", as she left. Tragically, when an outbreak occurred due to experimentation by the Alchemists, it is assumed that Carlo and Romeo later contracted the Petrification Disease and died, with Carlo passing before his friend due to an unknown cause.
Surprisingly, despite his neglect of the boy, Geppetto was devastated over his son's death. After learning that puppets regained their humanity as their Ego's awakened[1][2][3], Geppetto sought to bring his son back to life. He invented the P-Organ, which could absorb Ergo and transferred Carlo's Ergo into it. The puppet he created from his son's corpse, however, was too inefficient with Ergo and potentially destructive.
Geppetto stored the puppet away and created a new puppet named 'P' in a secret workshop within Krat Central Station. Transferring the P-Organ into P, Geppetto knew that P would collect massive amounts of Ergo which would enable the P-Organ to withstand the original puppet's body. He also made P the only puppet unbound to the Grand Covenant so that he would be able to collect Ergo unhindered. Additionally, Geppetto gave P an appearance almost identical to Carlo so that the collected Ergo would be refined in such a way that P would have a personality as close to Carlo's as possible.
Following P's completion, Geppetto departed and instigated the Puppet Frenzy so as to accelerate the collection of Ergo for the P-Organ. Despite his plans, Geppetto had not considered P as anything more than a puppet, yet as P became acclimated to the world, he proves Geppetto wrong by awakening his own form of humanity. As a byproduct of Carlo's ergo, P (according to Geppetto) had taken on some of his 'unsavoury traits'; such as mischievousness[4]. In a sense, Geppetto failed to realize that P had become his own human and could have been his son all along, however Geppetto's twisted mindset surrounding who Carlo was and 'should be' proved impenetrable.
Following the defeat of Simon Manus, Awakened God, if P allows Geppetto to take the P-Organ, a subservient and unfeeling version of 'Carlo' will be resurrected. He retains all the Ergo and power P had gained during his time in Krat. Geppetto then orders 'Carlo' to kill the remaining survivors in Hotel Krat, and replaces them with puppets in a twisted effort to create the perfect home for his son. 'Carlo' is notably different to his original incarnation, smiling warmly at his father and obeying him unquestioningly.
If P refuses to give up the P-Organ, Geppetto unleashes the Nameless Puppet on P. As the Nameless Puppet lunges to destroy P's heart, Geppetto, fearing for the P-Organ and Carlo's Ergo, steps in the way, taking the fatal blow in P's place. This allows P the opportunity to destroy what remains of Carlo's heart that resides in Nameless Puppet. As Geppetto dies, if P gained enough Humanity and helped Sophia, Geppetto notices P shedding tears over him. The inventor and former Alchemist finally realizes that P is more than just a puppet and has become human through his experiences: thus proving Geppetto wrong.

- His name is a reference of the original author of the book, Carlo Collodi.
- The name was also used in Guillermo del Toro's depiction of Pinocchio.
- If players choose to give Geppetto P's heart and resurrect Carlo, they will take control of him for the rest of the game. Carlo plays identical to P and also has access to a Legion Arm. However, the Legion Arm stops twitching when playing as Carlo. The survivors at Hotel Krat are also converted into obedient puppets who repeat the phrase "Your wish is my command Sir Carlo, what would you like to do?".
- Despite becoming human again, Carlo presumably still acts as a puppet due to his unwillingness to disobey Geppetto's order of killing the survivors at the hotel. This could be due to the puppet parts incorporated into the Nameless Puppet, possible making Carlo a kind of cyborg.
- Carlo and P share nearly identical physical features, with the sole distinction being that P boasts freckles and blue eyes, whereas Carlo lacks freckles and possesses brown eyes.
